The Union Dues Check-Off and Authorization Form is an Adobe PDF form which may be filled out either on-line in a web browser or printed and filled out by hand. When completing the form on-line, the bottom portion of the form will automatically complete itself. The bottom portion of the form is the employee's copy while the top part of the form should be submitted to Human Resources.
