This form must be turned in well enough in advance of the trip (at least 5 business days) to allow for approval and processing.
PLEASE NOTE: The override will be put in place and the pharmacy will be able to fill your prescription 5 days prior to your departure. Please plan to pick-up your prescription within that timeframe.
Please complete the form in full as all the information will be required to obtain an extension approval.
Return the form to Human Resources in McClain Hall 108. You will be notified when and if the extension is approved.
